TO: THOMAS WRIGHT and all other unknown putative fathers

You are hereby notified that on February 5, 1998, a female minor child named TaKara Mae Martin was born to Tammy R. Simpson (formerly Martin). A petition for Step-Parent Adoption of this child was filed by James W. Simpson Jr. in the Superior Court of Carroll County, in Case No. 14A019. If you are the biological father, you are to make known any objections you may have to this adoption no later than thirty (30) days from the last day of this notice in the Carroll County Superior Court before the Honorable Jack Kirby, Judge of Superior Court, at the Carroll County Courthouse, 323 Newnan Street, Carrollton, Georgia 30117.

You will lose all rights to the child and will neither receive notice nor be entitled to object to the adoption of the child unless, within thirty (30) days of receipt of this notice you file a Petition to Legitimate the child under O.C.G.A. §19-7-22 or an acknowledgement of legitimation pursuant to O.C.G.A. §19-7-21.1; and give notice of the filing of Petition to Legitimate with the Superior Court of Carroll County in the aforementioned civil action and send a copy to the below listed counsel for the Petitioner.
