Take notice that the Randolph County Department of Human Resources filed a petition in the Juvenile Court of Randolph County to terminate your parental rights of and concerning Naomi Burkes, Maquan Burkes, and Denasia Nicole Burkes. You must appear and answer the petition on or before the 26th day of September 2005, or a decree may be entered terminating your parental rights accordingly.

You have the right to be represented by an attorney of your choosing. If you cannot afford an attorney, the Court will appoint an attorney to represent you if you are determined by the court to be indigent.
